Pelvic Health During Perimenopause & Menopause
Changes during perimenopause and menopause can affect much more than periods or hot flashes. Some people notice new bladder urgency, leakage, constipation, vaginal or vulvar discomfort, pain with intimacy, prolapse symptoms, or changes in strength and recovery. These concerns deserve an individualized assessment rather than being dismissed as “just aging.”

People describe these concerns in many different ways. You may notice:
New or increasing urinary urgency, frequency, leakage, or recurrent irritation
Vaginal or vulvar dryness, burning, discomfort, or pain with penetration
Pelvic pressure, heaviness, or symptoms associated with prolapse
Constipation, straining, or bowel leakage
Changes in pelvic floor strength, relaxation, coordination, or exercise tolerance
Joint, tendon, hip, back, or pelvic discomfort that affects movement and confidence
You do not need to match every item on this list or arrive with a diagnosis. Symptoms can have more than one cause, and part of the first visit is deciding what pelvic therapy can reasonably address.

Pelvic therapy may address muscle tension or weakness, breath work and pressure management, bladder and bowel habits, prolapse-supportive strategies, mobility, strength, and graded return to activity. When dryness or tissue changes contribute to discomfort, therapy can reduce protective guarding and improve movement, while a medical provider evaluates whether vaginal moisturizers, lubricants, local hormone treatment, or other medical options are appropriate. Pelvic therapy does not treat osteoporosis, hormone imbalance, or metabolism itself; those concerns belong with the appropriate medical clinician.

Your first visit includes your symptom timeline, menopause stage if known, medical history, medications, exercise, sleep and stress context, bladder and bowel habits, and personal goals. Assessment may include breath work, mobility, hips, back, abdomen, balance, and the pelvic floor. Internal assessment is optional. Care is adapted for comfort, tissue sensitivity, and your preferences.

This service may benefit adults in perimenopause, menopause, or later life who have bladder, bowel, pain, prolapse, intimacy, core, or movement concerns. It is also appropriate for people who entered menopause after surgery or medical treatment, with coordination from their medical team.

Are these symptoms just part of aging?
They may become more common with age and hormonal change, but that does not mean you must ignore them. An assessment can identify modifiable contributors and when medical evaluation is needed.
Can pelvic therapy treat vaginal dryness?
Therapy does not replace medical treatment for tissue changes. It may help associated muscle guarding, pain, mobility, and return to comfortable activity. Your medical provider can discuss hormonal and nonhormonal options.
Can I exercise with prolapse?
Most people can remain active with appropriate symptom monitoring, breathing, pressure strategies, and progressive loading. The plan should be individualized rather than based on a blanket list of forbidden exercises. Our goal is to keep you enjoying the activities you love and feeling confident in your body.
Is internal treatment required?
No. Internal assessment or treatment is optional and based on your goals and consent.
